MIDDLE SCHOOL WINTER II  SPORTS PHYSICALS
Will be held on  March 7, 2024 in the MS Nurses Office at 9:30am
Students must have their Interval Health History Form on FamilyID prior to this physical.

You may download all required forms off the Athletic Website under: "Required Paperwork for Athletic Participation".  Please make every attempt to get a physical from your own Physician prior to the start date. 
Please use the NEW NYS PHYSICAL FORM located under the link "Required forms for participation."

*The Sports Physical is valid for one full year*

The School District provides Seasonal Sports Physical Exams to those students who require one prior to the start of the respective season, at no cost. These are for Athletic Participation Examinations only.  These exams are administered by Dr. Karl Friedman- District Chief Medical Officer/Physician.

It is required that each student athlete submit a sports physical, medical questionnaire and parental consent form in order to participate in interscholastic athletics.  All necessary forms can be obtained Online (District Website), Athletic Office (Rm. 157- HS), nurses office or PE Offices at MS.  All completed forms must be submitted on FamilyID.

In addition to the medical, it is the student's responsibility to complete a parental consent form and Concussion Form. These forms must be completed no fewer than five days prior to involvement in a sport.
